If you prefer to complete your own application form, we offer an online service, whereby one of our Legal Advocates can read and review this – along with up-to thirty pages of medical or similar evidence to ensure that this meets the criteria for the benefit and provide feedback by email, on our own, specially drafted feedback document.

 

**Please note – this form checking service is an entirely online service and you will not receive any phone calls in relation to this.**

 

You will need to provide us with a clear PDF scan of your completed form and evidence. Instructions on how to do this will follow on booking.

 

You will be contacted by email on or before the time of the appointment to confirm if we have received your documents and then you will receive feedback on each of your answers, by email, within three working days of the appointment.
